Step 1 : Read this entire form as it contains important information.

Step 2 : Fill out the "Contact Information" section of the form with your legal first and last name.  Note that the name you provide will be reflected on your certificate.

Step 3 : Select "Certificate of Graduation" if you would like to receive your certificate at the end of the current trimester or "Certificate of Graduation - Expedited" if you would like to have your certificate mailed within 2 business weeks.

Step 4 : Select "Quantity" of " 1 " of your chosen certificate.

Step 5 : Fill out your payment details.

Step 6 : Click "Submit"
